However, two widely publicized outbreaks—where infectious agents were transmitted through gastrointestinal (GI) endoscopy—have underscored the urgent need to address potential safety gaps across the entire endoscopy care continuum.
For three consecutive years, the ECRI Institute has included cross-contamination from flexible endoscopes on its list of the Top 10 Health Technology Hazards—a clear indication that improperly reprocessed endoscopes remain a serious and persistent threat to patient safety.

- Failure to follow established reprocessing guidelines
